Who can extend

  • S2 visa holders — extensions are a standard feature of the S2 visa, typically granted in increments related to the treatment plan.
  • Other visa holders — extension is possible in many cases with a valid reason (medical treatment is a recognized reason) and supporting documents.
  • Visa-free entrants — generally cannot extend. If there's any chance your stay may run long, enter on an S2 visa instead.

The golden rule: apply at least 7 days early

Extensions must be requested before your current stay expires, and the standard guidance is to apply at least 7 days in advance. Overstaying, even by a day, creates fines, record problems and potential entry bans — apply early, even if you might not need the extra time.

Where and how to apply

  1. Go to the Exit & Entry Administration (出入境管理局) office covering your city of residence — not the embassy.
  2. Bring your passport, current visa, and a reason and supporting documents (see below).
  3. Complete the application form (translation assistance is usually available, or bring your hospital's help).
  4. Pay the fee and wait — processing typically takes a few business days.

Documents that make an extension sail through

  • A letter from the hospital updating the treatment plan and estimated completion date.
  • Updated medical records or discharge/continuation notes.
  • Proof of accommodation and, in some cities, proof of funds for the extended stay.

Your hospital's international department handles extension letters regularly — ask them early, they know the format the Exit & Entry office expects.

Don't travel outside China to "reset". Re-entering on visa-free entry may not grant the same rights, and some routes create complications. In-country extension is the clean path for ongoing treatment.

Frequently asked questions

How long can an S2 extension be?

Extensions are usually granted in line with the treatment plan — commonly 30 days at a time, and repeatedly renewable while treatment continues with hospital documentation.

What happens if I overstay?

Overstaying results in fines per day, possible detention for long overstays, and can affect future visa applications. It is avoidable — apply for the extension before expiry.

Can my family's visas be extended too?

Yes — accompanying family members on S2 visas can extend alongside the patient with the same documentation plus proof of relationship.
